Subscription Services Cancellation and Refund Policy

1. Subscription Services
1.1 Enrollment
Your membership in a Le-Vel Rx subscription program ("Subscription Service") begins when you complete the applicable intake process and your initial payment is successfully processed.
1.2 Services Included
Subscription Services may include access to telehealth consultations, healthcare providers, prescription medications, if prescribed and appropriate, care coordination, educational resources, support services, and other benefits depending on the specific program selected.
1.3 Medical Eligibility
Participation in a Subscription Service is subject to medical eligibility as determined by an independent licensed healthcare provider. If a participating healthcare provider determines that you are not medically eligible for the selected program before any prescription medication has been dispensed, you will receive a full refund of amounts paid for the applicable Subscription Service.
1.4 No Guarantee of Results
Individual results may vary. Le-Vel Rx does not guarantee any particular outcome, result, weight loss, health improvement, or treatment success through participation in a Subscription Service.
2. Cancellation Policy
2.1 Cancellation Requests
You may cancel your Subscription Service at any time by submitting a cancellation request through your online account or by contacting support@le-velrx.com.
2.2 Notice Requirement
To avoid your next recurring charge, cancellation requests must be received at least seventy-two (72) hours before your next scheduled billing date. If a cancellation request is received less than seventy-two (72) hours before a scheduled billing date, the upcoming charge may still be processed and the cancellation will become effective at the conclusion of the following billing cycle.
2.3 Access Following Cancellation
Following cancellation, you will continue to receive Subscription Services through the end of your then-current billing period.
2.4 Chargebacks
Initiating a chargeback does not automatically cancel your Subscription Service. Members remain responsible for following the cancellation procedures described in this Policy.
3. Refund Policy
3.1 Medical Disqualification
If a participating healthcare provider determines that you are not medically eligible for the selected program before any prescription medication has been dispensed, you will receive a full refund of amounts paid for the applicable Subscription Service.
3.2 General Refund Policy
Except as expressly provided in this Policy or as otherwise required by applicable law, Subscription Service fees are non-refundable.
3.3 Prescription Medications
Federal law generally prohibits the return of prescription medications once they have been dispensed or shipped to a patient. Accordingly, prescription medications that have been dispensed, shipped, or delivered are not eligible for return or refund except where required by law.
If a refund is approved, it will generally be limited to the most recent billing cycle and will not apply to prior billing cycles.
3.4 Damaged or Incorrect Medications
You should inspect all medications immediately upon receipt. If a medication appears damaged, defective, incomplete, or incorrect, you must notify us through the Le-Vel Rx platform or customer support within twenty-four (24) hours of receipt so that the matter may be reviewed and coordinated with the applicable pharmacy and care team.
Upon verification:
- Damaged medications may be replaced by the dispensing pharmacy; and
- Incorrect medications may be replaced by the dispensing pharmacy.
4. Frequently Asked Questions
4.1 What Is Included in My Subscription Service?
Subscription Services are offered for a single recurring fee and may include telehealth consultations, healthcare provider evaluations, prescription medications, if prescribed and appropriate, laboratory services, care support, patient assistance resources, and other program benefits. Specific services and benefits may vary by program.
4.2 What Happens if I Become Ineligible for Continued Participation?
If a participating healthcare provider determines that you are no longer medically eligible for continued participation in a Subscription Service, your participation may be discontinued. Any refund eligibility will be determined in accordance with this Policy and applicable law.

5.2 Services Rendered
Except as expressly stated herein, you remain responsible for all fees associated with healthcare services, consultations, evaluations, laboratory services, medications, and other services rendered. This Policy governs Subscription Service cancellation and refund rights only.
5.3 Healthcare Decisions
Healthcare providers independently determine treatment eligibility and medical appropriateness. Le-Vel Rx does not guarantee that any individual will qualify for treatment, receive a prescription, or achieve any particular outcome.
